The DoH WA is seeking a highly experienced respondent with strong leadership and a high level of understanding of the Australia primary care network to design and conduct an evaluation of the Nurse Practitioner and Team Based Primary Care Pilot over 18 months.

It is expected that the successful respondent (evaluation team) will work with WAPHA to ensure each practice team is supported in wrapping evaluation around their initiatives and in sharing evaluation findings (e.g., the allocation of an external evaluation team member to each pilot site team, to provide an evaluation liaison function).

In addition, we would also expect the successful respondent to participate in any pre pilot inductions for NPs or PCPs to facilitate the evaluation to become embedded at each practice, with teams gradually building their evaluation capacity over time with a commitment to high quality data collection and evidence building.

The objectives of the evaluation are to:

• Report the progress and achievements of the pilot in meeting the objectives to increase access to primary care.
• Determine the overall effectiveness of the pilot to deliver its intended short, medium and long-term outcomes and the extent to which the initiative is contributing to increased capacity within primary care in Western Australia, particularly in under serviced and vulnerable populations.

The evaluation will:

• Include an analysis of the change to availability of NPs through the placement of practitioners in primary care practices and its impact on under serviced and vulnerable populations in Western Australia.
• Investigate where gaps in NP provision still exist and any factors that may influence a primary care practices decision to participate.
• Assess the impact of the funded NP services in primary care practices.
• Assess the impact of newly established services that were established through the pilot to increase NP participation in in Primary Care.
• Assess the ongoing viability of NPs in primary care and identify risks for ongoing service delivery.
• Determine enablers for NP service sustainability among a number of team based primary care models including specialist settings.
